Advanced Analysis of Facility-Based Maternal Mortality Data Using Full-Set BPJS Health Data and the Transition in the Era of Digital Health Transformation within the Government of Indonesia–UNFPA Cooperation Program for the 10th Cycle (2021–2025)

Executive Summary

Reconstra, as the consultant team, aims to strengthen Indonesia’s capacity to monitor and accelerate the reduction of maternal mortality by producing reliable and routine estimates of the Maternal Mortality Ratio (AKI) and Case Fatality Rate (CFR) using facility-based data. The project integrates Maternal Perinatal Death Notification (MPDN) and National Health Insurance (JKN) data through the Capture–Recapture (CRC) method to generate more accurate national and subnational estimates, while identifying systemic gaps and formulating evidence-based policy recommendations.

Under the collaboration of BAPPENAS and UNFPA (2022–2023), Reconstra is responsible for the end-to-end technical implementation of the study. This includes designing the methodological framework; conducting data cleaning, harmonization, and linkage between MPDN and JKN datasets (2020–2023); applying the CRC statistical approach; and producing national and provincial-level AKI and CFR estimates. Reconstra also leads data validation exercises, facilitates expert consultations and stakeholder workshops, conducts system and regulatory reviews, and prepares technical reports and policy briefs to support institutionalization of the estimation model.

The project seeks to establish a sustainable, replicable estimation mechanism to support routine monitoring aligned with the RPJMN 2024 and SDGs 2030 targets. Its expected impact includes improved data completeness and accuracy, strengthened cross-institutional coordination, enhanced data governance, and more targeted maternal health interventions at national and subnational levels. Key collaborators include BAPPENAS, UNFPA, the Ministry of Health (MPDN), BPJS Kesehatan (JKN), and subnational governments.
